PATIENT INFORMATION LEAFLET Changes to regulations for prescribing Gabapentin and Pregabalin
In order to register at Northgate Medical Practice, you need to live in the area covered by this Practice, as shown in the map via the Practice Area link on the right. You should bring your NHS medical card, with your new address completed and the document signed, to the surgery where you should book an appointment with our Nurse for a Health Check. If, however, you do not have a medical card, please ask the Receptionist for a registration form and provide proof of your current address.
As part of our ongoing service to patients:- If you are aged under 75, where the Doctor has not seen you for three years or more and you request an appointment, an appropriate check-up will be provided. Information for patients:
This practice has been accredited to train doctors intending to become general practitioners.
The practice needs to be regularly inspected and only doctors approved for this purpose will carry out the assessment.
Sometimes the doctor may wish to video record their consultations and patients may therefore be asked to sign a letter of autorisation to permit this. Your permission will always be requested beforehand.
